REGISTRATION

Please register by clicking below.

Your personal information will be stored securely. Registration costs are $250 per player and $1,000 per foursome team. This cost includes green fees, cart, a boxed lunch, and a cookout dinner.

Please make checks payable to Good Samaritan Health Center of Cobb with "Charity Golf Tournament" in the memo. You may also select to "Defer Payment" and pay at registration the day of the event.

 


ATTIRE

Be sure to wear comfortable clothing, so you are ready for action. You may want to bring rain gear, too.

FOOD

We will provide a boxed lunch at the shotgun start and a early dinner cookout during the awards ceremony.
